Just bring along your medical card, a utilitybill/ bank statement,i.e something that confirms your address, and photo ID please or in the case of a new baby the card obtained when registering the baby’s birth, and we will do the rest. Don’t worry if you have no card - our reception staff will give you the necessary forms to complete and explain the registration procedure. Once your registration has been accepted you will be offered a full health check with our Health Care Assistant or Practice Nurse.
